Contact the Community Health Center or First Nation and Inuit Health Branch local office at 807-345-1443 (phone) 807-343-9196 (fax). This must be done two weeks prior to appointment date.
The following information will be required to process the application through NIHB.
The information provided is faxed to NIHB local office that processes it for approval. Approval or denial of an application is forwarded to Fort William First Nation. Client is informed of decision by Health Centre Staff.
Medical transportation benefits include ground, water and air travel, meals and accommodations.
